Deakin University (Deakin) is collecting the personal information on this form for the primary purpose of registering you to participate in the TransformUs Program and access Program resources. Deakin will use this information to communicate with you about your participation in the Program, for evaluation of the Program and for Program-related research, if you have subscribed to receive updates. Deakin will not disclose your personal and medical information to any third party without your consent.

You are not required to provide the information requested, however if the information is not provided Deakin may not be able to register you to participate in the Program.
